Welcome to our practice!

After you have made your first appointment, please download, print, and complete the Patient Forms located below.

On the day of your first appointment, please arrive 15 minutes early with the completed Patient Forms, a list of current prescription and/or over-the-counter medications, including dose and frequency, and any recent test results, xrays, or relevant records for the doctor to review.

If you need directions, please see the Locations page on this website.  If you need a referral from your primary care physician, please make the necessary arrangements.

Also please bring a copy of your insurance card(s) so that we can bill your insurance carrier.  All co-pays and/or deductibles are due at the time of service.
